New Dental Patient Information – Melbourne, FL
You’re One Step Closer to Relieving Your Dental Pain
Are you preparing for your upcoming appointment with Dr. Scott and our team at Associated Endodontists of Melbourne? We ask that you please take a look at the information on this page, and if you have any questions, give us a call. Below, you’ll find our new patient packet, what you can expect during your visit, and our in-network dental insurance plans. We look forward to helping ease your discomfort and providing you with the fast and effective treatment you need!
Your First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history will require an evaluation and a second appointment to provide treatment on another day. Please assist us by providing the following information at the time of your consultation:
- Your referral slip.
- A list of medications you are presently taking.
- If you have dental insurance, your insurance card. This will save time and allow us to help you process any claims.
- You do not need to bring any existing X-rays. Our office will take specific X-rays as Dr. Scott needs to see the exact angles of the root.
- Do not take any Advil, ibuprofen, or pain medication prior to midnight the night before your initial consultation appointment.
IMPORTANT: A parent or guardian must accompany all patients under 18. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings.

Dental Insurance
We’re happy to accept most major dental insurance plans to make sure you can get the care you need without ruining your monthly budget. We’re in-network with Delta Dental, Humana, Careington, Lincoln Financial, Best Life, United Healthcare, GEHA, Solstice, and Connection Dental Network. If you have a PPO plan that isn’t listed here, our team can still file an insurance claim on your behalf to help maximize your reimbursement.
CareCredit Financing & Other Payment Options
In addition to accepting dental insurance, we’re happy to offer other easy ways to pay as well. We accept all major credit cards and provide a cash-discount for those who pay upfront. If you’re uninsured or underinsured, we also work with third-party financier, CareCredit, to offer low monthly payment plans at little-to-no interest.
